For now, the denizens of Genoa City believe that Ian Ward is really and most sincerely dead, but The Young and the Restless fans know that is just not true.
Ian’s Not-So-Final Hurrah

When Victor shot up out of his chair with a bullet wound to the belly and tackled Ian, fans everywhere cheered, especially when the gun went off. Ian allegedly died on an ambulance gurney with Mariah saying a final goodbye to the monster who had kidnapped her and raised her in a cult.
However, when Ian was in the ambulance, we saw his eyes suddenly flutter open indicating that he was not dead at all. Did Ian manage to make his escape and will Ray Wise be back so his alter-ego can torture the Newman clan and Mariah once again?
Ray Wise Rules Nothing Out

Wise described to Soap Opera Digest how that final scene came about — a shocking final scene that leaves more questions than answers.
“We did have a conference about how to do it and we actually did it both ways — we did it very slowly, and then we did it quickly,” Wise said. “And we decided the quicker way was best, with a little burst of air coming out of my mouth.”
Wise loved all the elements he could play this time around on Y&R. He returned last fall after a decade away from the show.
“We had no restrictions on where we could go, really,” he said. “I mean, we had to stay within the bounds of network taste, but we could go any which way, and we tried to keep our sense of humor about it. We had a very good rapport, Colleen and I, and she was just the perfect acting partner for me on the show.”
Since Ian did open his eyes, Wise looks forward to possibly reprising the role one day.
“Well, you can’t keep a good villain down, you know? You just can’t do that! And I was very happy that they kept the door open for Ian to make a return at some point.”
